Submitting A Service Request


Step 1

Contact your agent / customer service to obtain the required form(s)


Step 2

Complete all the fields in the form accurately


Step 3

Provide all required offcial signatures


Step 4

Provide the necessary identity documents


Step 5

Submit the completed form(s), supporting documents and / or payment (if any) via your agent or to a Prudential branch

Important Notes Prior To Submission:

  1. Ensure all fields are completed and your signature is executed in the same manner as in the Proposal Form or your latest Signature Declaration Form.
  2. Do ensure completeness of consent from your policy trustee(s) if there is any.
  3. Attach relevant supporting documents (if any), e.g. the latest medical report and proof of identity. You can contact your agent on the relevant supporting documents needed for your application.
  4. Submit the completed form and supporting documents to us via your agent or at your nearest branch.
  5. Submit all necessary payments. You may pay by Cash/Cheque/Credit Card at your nearest branch
